As of 1st April 2025, there will be an administrative change to the way the five GP practices within Braintree Primary Care Network (PCN) work together.
Blandford Medical Centre, Blyth’s Meadow Surgery and Mount Chambers Surgery will directly manage the PCN but will work closely with Church Lane and Silver End practices to ensure all patients continue to have access to the full range of PCN services they currently do.
This means that patients from all five practices will still benefit from the following PCN services:
• Enhanced Access (all appointments covering 6.30-8.00pm Monday to Friday and Saturdays 9.00am-5.00pm will continue to be access your registered practice);
• Support to care home residents;
• Medication reviews and advice;
• Access to a broad range of healthcare and support professionals: pharmacists, nurse practitioners, physician associate, paramedics, physiotherapists, care coordinators, social prescribers and mental health workers.
These PCN services are in addition to those already provided directly by each of the practices to their registered patients, such as medical advice, vaccinations, screening, and management of long-term conditions.
Patents should not notice any difference as a result of this change, and all practices remain committed to jointly delivering the same high standards of service our patients rely on.
